The tab menu retains the closed windows between restarts, but maybe only if you have "Continue where you left off" set for startup.. It's the button next to the minimize button. The next time this happens, just reopen Opera and reopen the closed window from the tab menu button, or from O Menu -> History.

That happens when you hold Ctrl as you press Enter. There is currently no way to disable or change this behaviour.
They have given us customisable keyboard shortcuts for everything except this very annoying one.

I knew there was something about it! I don't remember pressing ctrl when this happened so IDK what caused it but it is nice to know that I was not going crazy.
