The exceptions are stored in the "Preferences" file in the roaming profile folder.
When using the allow button on the toolbar that pops up on adobe.com for the About Flash page, Opera generates this:

When adding sites manually to the exceptions list, an entry only seems to go in the "plugins" object and not both "plugins" and "permission_autoblocking_data".
Just loading the page though before right-clicking the plug-in area, choosing "Enable Flash" and clicking "allow", Opera generates this:

While Opera is closed you can try to play with these settings or remove them to see if you can get Opera to load flash for a site. Not a practical workaround or anything. But, playing with them may reveal the problem.

It's a lot of help.
Now we stay to see if some developer take a look here to try to fix this instead we need to use workarounds.
I personally don't have the necessary skils to do something like this but with your contribuition I guess it's a big clue to solve this. -

@burnout426 To solve this problem I simple delete the file "Preferences" that are located in different places depending if in Linux or Windows (I don't have a Mac).
The issue is that for me I need to reinstall the addons, but the bookmark and history are preserved.
So in my GNU/Linux installed system the file preferences stay in a path like:
/home/username/.config/opera
In Windows installed system the path is:
C:/Users/username/AppData/Roaming/Opera Software/Opera Stable -

@msxmaniac Please, is this the mentioned above "preferences" to delete(in this following screenshot) and now does Flash Player work in all websites using flash player, either audio- or video file?

If you have many extensions I encourage you to try this first.

Hmmm....
It works for all the sites I've tested.
I cannot help anymore on this, the only thing maybe is to use an oldest version, but latest O48 fixed all my OStable Flash problems.
Open all the extensions websites you have and save them as a SpeedDial folder if you try the other workaround - delete Preferences file -.

@zalex108 I deleted the "Preferences" in my second PC but nothing happened, the problem exists, only in my first PC(without deleting its Preferences file) only one of the websites using flash player(it has only audio files) started working!! and flash player in other website does not work yet.

@shahramop If you have Adobe Flash properly installed (test with Chromium first) it's exactly the file to be renamed/deleted from this folder.
The only side effect I had was having to reinstall all the addons.
And of course the site with flash content needs to be inserted into the flash content execution exception list that is in the browser settings. -

I've been playing with chrome://site-engagement more. Setting a site to 0, pressing enter and refreshing that page does indeed remove the site from the list.
Also, setting a site to 100 (and pressing ENTER to apply it) and refreshing the site's page often allows flash to work without even adding an exception to the list. I say "often" as it doesn't work all the time. Sometimes I have to close out of the tab for the site and open it up again. Other times I have to repeat that stuff a lot of times or refresh the site page a bunch of times. Then it works, again without adding an exception.
So, maybe try going to chrome://site-engagement and setting all the sites to 100 (making sure to press ENTER each time). Then, for sites where that still doesn't help, keep reloading the site for a while just to see if you can get it to work.
If a site isn't in the list, go to the site first and then it'll show up. If it still doesn't though, you'll have to try the exceptions list. Then it should show up in the site-engagement list where you can then set it to 100.
Not a solution. Just something to mess around with. Also, once it works for a page, I'm not sure if it keeps working (after a restart for example) yet. But, it looks like it persists.

@shahramop i had the same issue. what i did here was, that i simply clicked on "nu controleren" and downloaded flash player again. when i went to facebook games, opera now asked me to allow flash, which i did. Nothing happened here, BUT: using the site settings bar now, setting plugin+NPAPI plugin to allow, enabled the flash and i could display game content without problems.
