Question about the + icon on bookmarks bar.

Why does Opera have the + at the left of the bookmarks bar duplicating what the heart does on the address bar?

Because they do different things. One adds a new entry to the bookmark bar and the other adds the page to bookmarks.

Also, not everyone uses the bookmarks bar.
