      The ad looks like this:
      http://puu.sh/tankQ/b55adb904a.png

The opera adblock doesn't block it but the adblock plus extension does. Is it possible to manually block it with custom block list in Opera settings?

I tried copying and pasting the web address into custom block lists and it says "Import failed: Parse error. Try again." Any suggestions? Hmm

You need to create a list using the Easylist format and include the address on it. And then use that list in 'custom block list'.
