and how to add so that email attachment dropdown have this custom option ?

I haven't seen any way to add a directory to the Open and Save dialogs ... though I suppose you could try using Set preference to change the saved location and see if that works.

Opera 12 (and hence Opera Mail) saves the last folder used in Preferences, and you should be able to change it with the Set preference command. Though some preferences require a reboot, so possibly it won't really work.
