@lineweight Your first item says to add Open in external app" to the page menu, meaning that once a page has opened in Opera you could select to open it in the app from there. That allows you to choose for each page, rather than always using the app.
The second option - when on any YouTube page click on the icon in front of the address in the address bar. At the bottom of the panel that pops up will be a link to site settings. Changing anything there adds the current site (YouTube) to site settings, so just enable "Always open in app".

With the first option, I see it lets me choose to do this "always" rather than just this time. And choosing always adds twitter to the list of sites visible in the second option.

It's poor that there's (as far as I can see) no help documentation for Opera that would let me find this out for myself.
