In Opera 20 there is no Menu Toolbar. You can see the menu though by just clicking on the Opera Menu to the top far left of the browser.
Control Shift E shows your extensions (it's a keyboard shortcut for clicking on the Opera menu at the top far left of the browser, and then clicking on extensions.
Control H shows your history (or click on the Opera Menu and then click on history).
You can add a personal bookmarks bar by going Alt P (a keyboard shortcut to clicking on the Opera Menu, and then Settings), then click on Browser (if it isn't already in bold), and then under User Interface, put a check in Show the Bookmarks Bar.
