Open options (Alt+P) and go to 'On startup'. Look at what your set pages are. Also have a look in your extensions and plugins (two separate pages) and make sure its not in there also. Also, when you say 'most' anti-adware, did that include Malwarebytes, Spybot and ADWCleaner?
Do you know where you picked it up from to begin with?

Did you check your set pages in Opera Options? How about the Plugin and Extensions pages? Did you try malwarebytes? Also, did you check Programs and Features in the Windows Control Panel?

Definitely use Malwarebytes. I have used it on other machines and works like a charm.

Do you have Opera shortcut somewhere? (Sure you do.) Check shortcut properties - some badwares attach themselves just to command line to run executable.

+1 to what donq wrote. Also, double-check your search engines... mystartsearch is known to install its own search engine and try to make it default (not so easy in Opera, but still, make sure it hasn't added itself to the list).
