Speed Dial Icons Changed When Restarted, & Titles don't Match the Websites!

I just rebooted my computer, and opened Speed Dial, and some of my usual icons are gone, replaced by a bunch of websites I've visited, but never added. And then I see many have one website as the title, yet another as the address, it's totally weird, it's like someone overturned a jig saw puzzle and everything's completely helter skelter! How the hell do I sort this out, and I don't feel like going into each one, fixing them, reinserting the ones that vanished, only to have the same thing happen again!

What in the world could have caused this? And is there any way to fix it, so that it won't keep happening?
