I think I figured it out. There were 3 tiny dots that blended in with my background wallpaper. When I clicked on them, it gave me the option to not show again. The fashion recommendations disappeared. 
Something must have updated and glitched. That would also explain the preinstalled speed dial tabs reappearing and intermixing with my own.
I'm getting the same spam in the speed dial screen.
Across the top, I get "Because you like fashion" along with 6 links to sites that I wouldn't take clothing from if it were free.
If I restart Opera or reboot the PC, it doesn't appear during the first visit to speed dial, but reappears after the 2nd visit.
At one point, a bunch of new spam links were mixed in with my normal speed dial links. I deleted them and so far they haven't returned.
This started happening today, my settings have not changed and I have confirmed suggestions and promotional speed dials, campaigns etc. are still Off .
