Last Pass Extension Missing but Says "Installed"

My last pass extension randomly disappeared. It isn't in the toolbar and won't automatically activate when I attempt to enter a password. If I visit the extension page it says "Installed" but it is not listed in the "My add-ons" page. At this point I can't install it or uninstall it, it's lost in limbo... Help?

Not sure this will help but I always install the Chrome extension which has received multiple updates while the Opera one has languished for nearly 7 months. I'd rather use the native extension but got tired of chasing down weird bugs that are almost always resolved with the Chrome extension. Good luck.
