Icon for add-to-home screen doesn't show

I'm trying to get my site icon to display when the page is added to the homescreen or the speed dial. Other sites have them - mine just shows a blue box with the site name.

I have included plenty of sizes/formats, including apple-touch-icon-180x180.png.
I have a working manifest.json file & browserconfig.xml.

What gives???
