In general this seems to work well, but one site, http://www.nationalpost.com/index.html, manages to display ads anyway. I wonder how they do it, and whether the Opera ad block can be modified or trained to block these too.

I think they use EasyList.
Using
Alternate DNS + HostFile (HostsMan) thanks to @Nabucco + Opera AdBlock + AdGuard

I updated my hosts file, added in Adsguard, but to no effect. I still see some annoying adds for Indochino suits; not bad looking suits, but I don't want to see them. Thanks for the suggestions however.

Try with Steven Black Hosts, in AdGuard there are some lists to subscribe, check for your navigation countries, and may be need it to manual block some ads.
