Always open links in a new tab, never a new window?

Is this possible?

There is no scenario in which I want a link to open in a new window (unless I specifically choose to do it myself).

I use cmd + left mouse click to open links in new tabs.

Shift + Middle-click to open links in a new foreground tab.

Middle-click = background tab.
