Would it not be advisable to have mybrowseraddon.com actually be able to submit a form to give extension authors some feedback? It just gives an error message, "the form could not be submitted", which is not very helpful. Some indication of what the cause of the error is might help. If it is incorrect programming for the CAPTCHA, then fixing that would probably make it work again.
