The white area in which it should reside is there, but there's no bar and the manual up/down buttons are greyed. This seems to happen randomly and always fixes itself after a refresh of the page.

This problem doesn't occur in incognito mode, and hasn't seemed to happen outside of Youtube; However, I've tried disabling all my extensions and adblock to no avail. Any ideas on what could be different between incognito and my normal setup?

This is on a completely new profile directory; I changed some settings and flags to what they were originally and saw the problem has occured again. I then reset all my settings and it still occurs.

It'd also help out in pinpointing what caused the problem if it starts again.

Before doing that you can try resetting settings and flags.

Can also be a setting and/or a broken profile.
