The answer is, Trend Micro no longer supports Opera. I just found out the hard way.

Maybe someone at Opera (hint hint) can get with someone at Trend Micro, and see if the support can be reinstated.

Anyway, if you have real time protection turned on, then it should check all downloaded stuff.
