<![CDATA[Have I been hacked?]]>Google searches keep asking me if I am visiting sites from outside the US, and I am seeing duplicate headings and text in Russian. What is going on here?

Since you're using Opera's VPN, and many people from Russian-speaking countries are too, it smartly determined that you must be Russian-speaking!

Not much to do about this apart from changing the language in your Google preferences. And maybe hoping that their algorithm will become a bit smarter still.
