I confirmed that this issue happen on Firefox 49.0 and Vivaldi 1.5.609.8 on macOS Sierra. So, I think the reasons of this issue is macOS Sierra.
Incidentally, another bug like this issue happen on Safari 10.0 on macOS Sierra. When I opened new window in macOS Sierra's full-screen mode, Safari opened new "tab"!
So, I think this issue connect with macOS Sierra's new feature "Tabs".

When I opened new "private" window in macOS Sierra's full-screen mode, Safari opened new normal private window. And when I opened new window in Desktop mode, Safari opened new normal window. -
